HMO/Freedom Network and HMO/Liberty Network3
Oxford has always offered traditional HMO plans for those who like to stay in-network and have their care coordinated by a primary care physician (PCP). Oxford's HMO plans allow Members to receive covered services from the in-network primary care provider of their choice. Oxford's Freedom and Liberty HMO plans offer:

In-network coverage only
Copayment for in-network physician office visits
PCP referral required for all specialist visits (except OB/GYN)

Note: HMO/Freedom Network is no longer available for large groups in New York as of January 2004

HMO Select/Freedom Network and HMO Select/Liberty Network3
These HMO Select plans are administered like our HMO plans described under HMO products, except Members do not need a referral to see a specialist. Additionally, Connecticut HMO Select Members do not need to select a primary care physician.

Note: HMO Select/Freedom Network is no longer available for large groups in New York, but will be available to large groups in New Jersey as of January 2004.

HMO LaurelSM
Oxford created the HMO Laurel for small employers in Connecticut that need more affordable options, without sacrificing quality. The HMO Laurel is a pre-packaged referral plan with in-network only coverage. All HMO Laurel Members have access to our Freedom Network of doctors and hospitals, which includes access to all Connecticut acute care hospitals. This plan also allows Members routine in-network preventive care, laboratory services and radiology services at no charge.

HMO Laurel Select
The HMO Laurel Select is also available to the Connecticut small group market. This plan is very similar to the HMO Laurel, except that a referral is not required to access in-network care. All HMO Laurel Select Members have access to our Freedom Network of doctors and hospitals, which includes access to all Connecticut acute care hospitals. This product also allows Members routine in-network preventive care, laboratory services and radiology services at no charge.

Oxford Exclusive PlanSM Metro
The Oxford Exclusive Metro Plan is an open access, in-network only product available with both the Freedom and Liberty Networks to small groups is New York. These pre-packaged plans contain a split copayment for PCPs and Specialists paired with hospital copayments for inpatient and outpatient services.

Oxford Exclusive PlanSM
The Oxford Exclusive Plan provides large groups with in-network only access without referral to the area's premier network, the Freedom Network and is available for large groups in New York. This pre-packaged plan contains a split copayment for PCPs and Specialists paired with hospital copayments for inpatient and outpatient services.

Point-Of-Service Products(in-and out-of-network)

Oxford became known for being an innovator in the healthcare industry by creating the first combined in- and out-of-network plan in the tri-state area. These point-of-service plans are our most popular product, giving Members the choice to go in- or out-of-network for care.

Claim form submission required for all out-of-network services
Deductible and coinsurance applicable to all out-of-network services

We understand that choice and flexibility are a top priority for many. So we created the Freedom Plan and the Liberty Plan and became the first company in the tri-state area to offer in- and out-of-network benefits within the same plan.

Freedom Plan® ClassicSM and Liberty Plan ClassicSM 3
Many people feel it is a good idea to have the guidance of a primary care physician (PCP) throughout the healthcare process, and for those consumers we offer Freedom Plan Classic. Requiring a PCP referral prior to most specialist visits also allows for lower premium rates. Freedom Plan Classic provides a great value, and offers:

Freedom Plan® MetroSM and Liberty Plan MetroSM 3
The Freedom Plan Metro and Liberty Plan Metro products offer healthcare coverage for New York small groups at a lower premium cost. Freedom Plan Metro Members have access to the Freedom Network and Liberty Plan Metro Members have access to the Liberty Network. Other attractive features of these plans include:

Freedom Plan® LaurelSM
Oxford created Freedom Plan Laurel for Connecticut small employers who are looking for affordable point-of-service products for their employees. To keep costs low we have developed pre-packaged plan design options that utilize cost sharing strategies such as split copayment and in-network hospital copayment. All Freedom Plan Laurel Members have access to our Freedom Network of doctors and hospitals, which includes access to all Connecticut acute care hospitals. This plan also allows Members routine in-network preventive care, laboratory services and radiology services at no charge.

Open Access Products / No PCP Referral Required

Understanding that some people prefer not to have to obtain a referral, we created open access/no referral plans to allow Members to visit in-network specialists directly.

Freedom Plan® AccessSM and Liberty Plan AccessSM 3
Freedom Plan Access and Liberty Plan Access do not require a referral for specialist visits. Instead specialist copayments are set at a higher rate than primary care physician (PCP) copayments since specialist visits cost more. The result is more affordable rates and plans that give Members greater flexibility to choose their own doctors and healthcare coverage options. Freedom Plan Access and Liberty Plan access offer:

Separate office visit copayments for in-network PCPs and specialists
Access to in-network specialists without referrals
In- and out-of-network coverage
Multiple prescription drug rider options

Freedom Plan® SelectSM 4 and Liberty Plan SelectSM4
These open access plans are administered like our point-of-service plans, except:

Members may self-refer to participating specialists and receive in-network coverage.
Members do not need to select a PCP with most of our open access plans. (Plans offered to New York groups require the selection of a PCP)

Freedom Plan® DirectSM and Liberty Plan DirectSM 3
Freedom Plan Direct and Liberty Plan Direct use in-network deductibles to provide flexibility and lower prices. Freedom Plan Direct and Liberty Plan Direct offer:

Freedom Plan® Value OptionSM
The Freedom Plan Value Option was created for both large and small groups in the Connecticut market, and allows Members to access Oxford’s renowned Freedom Network of doctors and hospitals without a PCP referral. By offering higher copayments and in-network deductibles for hospital and facility based services, Oxford is able to offer this product at lower price points.

Freedom Plan® Laurel SelectSM
Oxford created Freedom Plan Laurel Select for Connecticut small employers that need more affordable options that don't sacrifice quality. Consumer-Driven Health Plans

Oxford’s answer to consumer choice was to develop our point-of-enrollment plan which gives Members the control to choose the plan that best fits their healthcare needs.

Oxford Consumer Options SuiteSM
The Oxford Consumer Options Suite includes an electronic point-of-enrollment tool kit that offers clients the ability to choose from three Oxford plans with three price ranges: Freedom Plan Access, Freedom Plan Classic and Freedom Plan Direct. This tool kit was created in an effort to provide affordable health plan options to employer groups without sacrificing quality.
This tool kit is unique in its creation of an electronic plan designer for use with the Oxford Consumer Options Suite. Benefits of this electronic tool kit are:

Brokers and employer groups can customize their health plans to fix their per-employee expenses
Employers get more flexibility and a larger range of products from which to choose
Employees get more choice and have the ability to select a plan that best suits their healthcare needs

Oxford MyPlan Oxford MyPlan combines quality non-referral health plans with a unique Health Reserve Account (HRA) to help cover deductible and coinsurance costs. Funded by the employer, the HRA covers the first half of the employee's deductible. Oxford MyPlan puts the Member in control of their healthcare dollars yet provides all the quality and support needed to help deliver a better healthcare experience.

Oxford recognizes that you may have employees that live beyond our service area. Therefore, we’ve created our out-of-area product using affiliated networks.

Oxford USAsm
Oxford USA5 is available to multi-site employers through our affiliation with The First Health®Network6, a national provider network. Oxford can now offer flexibility to New York, New Jersey, Connecticut, Pennsylvania and Delaware employers who have employees living beyond our core service area. This will enable companies to offer the same quality health plans and responsive service Oxford is known for to their employees in other parts of the country.
