Oxford has always offered traditional HMO plans for those who like to stay in-network and have their care coordinated by a primary care
physician (PCP). Oxford's HMO plans allow Members to receive covered services from the in-network primary care provider of their choice.

HMO Select/Freedom Network and HMO Select/Liberty Network3
These HMO Select plans are administered like our HMO plans described under HMO products, except Members do not need a referral to see a
specialist. Additionally, Connecticut HMO Select Members do not need to select a primary care physician.
Note: HMO Select/Freedom Network is no longer available for large groups in New York, but will be available to large groups in New Jersey as of January 2004.
HMO LaurelSM
Oxford created the HMO Laurel for small employers in Connecticut that need more affordable options, without sacrificing quality. The HMO Laurel
is a pre-packaged referral plan with in-network only coverage. All HMO Laurel Members have access to our Freedom Network of doctors and
hospitals, which includes access to all Connecticut acute care hospitals. This plan also allows Members routine in-network preventive care,
laboratory services and radiology services at no charge.
HMO Laurel Select
The HMO Laurel Select is also available to the Connecticut small group market. This plan is very similar to the HMO Laurel, except that a
referral is not required to access in-network care. All HMO Laurel Select Members have access to our Freedom Network of doctors and hospitals,
which includes access to all Connecticut acute care hospitals. This product also allows Members routine in-network preventive care, laboratory
services and radiology services at no charge.
Oxford Exclusive PlanSM Metro
The Oxford Exclusive Metro Plan is an open access, in-network only product available with both the Freedom and Liberty Networks to small groups
is New York. These pre-packaged plans contain a split copayment for PCPs and Specialists paired with hospital copayments for inpatient and
outpatient services.
Oxford Exclusive PlanSM
The Oxford Exclusive Plan provides large groups with in-network only access without referral to the area's premier network, the Freedom Network
and is available for large groups in New York. This pre-packaged plan contains a split copayment for PCPs and Specialists paired with hospital
copayments for inpatient and outpatient services.
Oxford became known for being an innovator in the healthcare industry by creating the first combined in- and out-of-network plan in the
tri-state area. These point-of-service plans are our most popular product, giving Members the choice to go in- or out-of-network for care.

Many people feel it is a good idea to have the guidance of a primary care physician (PCP) throughout the healthcare process, and for those
consumers we offer Freedom Plan Classic. Requiring a PCP referral prior to most specialist visits also allows for lower premium rates. Freedom

Freedom Plan® LaurelSM
Oxford created Freedom Plan Laurel for Connecticut small employers who are looking for affordable point-of-service products for their employees.
To keep costs low we have developed pre-packaged plan design options that utilize cost sharing strategies such as split copayment and
in-network hospital copayment. All Freedom Plan Laurel Members have access to our Freedom Network of doctors and hospitals, which includes
access to all Connecticut acute care hospitals. This plan also allows Members routine in-network preventive care, laboratory services and
radiology services at no charge.
Understanding that some people prefer not to have to obtain a referral, we created open access/no referral plans to allow Members to visit in-network specialists directly.
Freedom Plan® AccessSM and Liberty Plan AccessSM 3
Freedom Plan Access and Liberty Plan Access do not require a referral for specialist visits. Instead specialist copayments are set at a higher
rate than primary care physician (PCP) copayments since specialist visits cost more. The result is more affordable rates and plans that give

Oxford plans with three price ranges: Freedom Plan Access, Freedom Plan Classic and Freedom Plan Direct. This tool kit was created in an effort
to provide affordable health plan options to employer groups without sacrificing quality.
